Medical Release Forms
We are making things a little easier for the students and their parents.
On one of the tabs above you will see "Medical Release Forms". This medical release form will be good from now until the end of the 2013 year. We will have copies of this form available at the Lighthouse. By filling this out, you will not have to fill out any other medical release forms for the rest of the year (besides the one for camp.)
